Aventura at Pembrooke is seeking a caring and dedicated Social Services professional to join our skilled nursing and rehabilitation team. This position plays a vital role in supporting residents and their families by addressing psychosocial needs, coordinating resources, and helping ensure a positive resident experience.

Responsibilities
- Assist with psychosocial assessments for residents upon admission and throughout their stay.
- Provide emotional support and counseling to residents and their families.
- Participate in care plan meetings and collaborate with the interdisciplinary team.
- Assist with discharge planning and coordinate community resources.
- Help residents and families navigate healthcare services and available benefits.
- Advocate for resident rights and promote resident satisfaction.
- Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
- Assist with resident adjustment, transition, and discharge needs.
- Build positive relationships with residents, families, staff, and outside agencies.
- Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred.
- Previous experience in a skilled nursing facility, rehabilitation center, long-term care, or healthcare setting is preferred.
- Excellent communication, organizational, and interpersonal skills.
- Compassionate, empathetic, and resident-focused.
- Knowledge of long-term care regulations and discharge planning is a plus.
