- Develop and maintain strong relationships with hospitals, physicians, case managers, social workers, discharge planners, and other healthcare professionals.
- Serve as a professional representative of the facility within hospitals and the local healthcare community.
- Conduct regular hospital rounds and maintain consistent communication with referral sources.
- Identify potential skilled nursing and rehabilitation referrals.
- Respond promptly to referral opportunities and work closely with the admissions and intake teams.
- Complete clinical assessments and bedside evaluations when appropriate.
- Assist residents and families in understanding their post-acute care options.
- Communicate clinical information and admission updates to referral sources in a timely manner.
- Follow referrals through the admissions process and maintain consistent follow-up.
- Build awareness of the facility's services, clinical capabilities, and specialty programs.
- Participate in educational presentations, provider meetings, community events, and outreach activities.
- Maintain accurate referral and marketing activity in the appropriate CRM/EHR systems.
- Identify barriers to referrals and communicate trends and opportunities to facility leadership.
- Collaborate with the interdisciplinary team to support census development and successful transitions.
- Previous experience as a Clinical Liaison, Hospital Liaison, Skilled Nursing Admissions Coordinator, or Healthcare Marketer within a skilled nursing or long-term care setting.
- Experience working with hospitals, physicians, case managers, discharge planners, and referral sources.
- Knowledge of skilled nursing, rehabilitation, long-term care, or post-acute healthcare services.
- Strong clinical assessment and communication skills.
- Excellent relationship-building, presentation, and networking abilities.
- Professional, compassionate, and customer-focused approach.
- Strong organizational and time-management skills.
- Ability to work independently while collaborating with clinical and administrative teams.
- Proficiency with Microsoft Office, CRM, and/or EHR systems.
- Valid driver's license and reliable transportation for hospital and community outreach.
