Aventura at Creekside is seeking an experienced, driven, and relationship-focused Admissions Director / Hospital Liaison to lead our admissions efforts and strengthen partnerships with hospitals, physicians, case managers, and community referral sources.

Key Responsibilities
Develop and maintain strong referral relationships with hospitals, physicians, case managers, and other healthcare partners.
Serve as the primary liaison between referral sources and the facility.
Coordinate and manage the admissions process from referral through admission.
Evaluate referrals to ensure appropriate clinical and financial acceptance.
Collaborate with the interdisciplinary team to ensure smooth resident transitions.
Conduct regular hospital and community visits to generate new referral opportunities.
Track admissions activity, referral trends, and census goals.
Participate in marketing initiatives, networking events, and community outreach.
Maintain compliance with all federal, state, and facility policies.
Qualifications
Previous experience in healthcare admissions, business development, hospital liaison, or clinical marketing required.
Skilled Nursing Facility (SNF), rehabilitation, long-term care, or hospital experience strongly preferred.
Excellent communication, relationship-building, and negotiation skills.
Strong organizational and time management abilities.
Ability to multitask and work independently in a fast-paced environment.
Proficiency with Microsoft Office and electronic medical record systems.
Valid driver's license and reliable transportation for local travel.

Salary : $82,845 - $128,900
