Position Summary:
Aventura Skilled Nursing Facility is seeking an experienced MDS Coordinator (LPN/RN) to lead resident assessments and ensure accurate, timely MDS submissions. This key role supports quality care, regulatory compliance, and interdisciplinary care planning.

Qualifications:
- Active LPN or RN license (in good standing)
- Minimum 2 years of MDS experience in long-term care
- Strong knowledge of RAI process, PDPM, and reimbursement
- Detail-oriented with excellent organization and communication skills
- Team player with ability to work independently
Key Responsibilities:
- Complete and submit accurate MDS assessments (MDS 3.0)
- Ensure compliance with CMS and state regulations
- Lead care plan meetings and coordinate IDT collaboration
- Oversee CAAs and individualized care plans
- Audit documentation to support coding accuracy
- Track assessment schedules and maintain compliance
- Support audits, surveys, and quality improvement initiatives
