Key Responsibilities

Help develop and carry out care plans for residents.

Supervise and assist Certified Nursing Assistants (CNAs).

Monitor residents' health and safety, including falls and skin care programs.

Provide direct patient care and assist with daily medical needs.

Complete and update resident evaluations and documentation.

Support overall care goals and perform other duties as needed.
Qualifications

Current Licensed Practical Nurse (LPN) license in good standing.

Experience in skilled nursing or rehabilitation preferred.
