Avante at Ormond Beach is seeking a Wound Care Nurse to join our team of long-term care professionals. This is a vital role in a facility with a positive community reputation, a history of successful outcomes, and an experienced cohesive team. This is a 133-bed skilled nursing facility, with an In-house therapy department.

Description:
To provide weekly monitoring of existing pressure ulcers, with recommendations for treatment changes in the absence of improvement. Recommending and implementing preventative interventions to prevent skin breakdown for immobile and inactive residents in accordance with state and federal regulations. To direct and supervise the restorative needs of all residents, in cooperation with the Rehab Director and Director of Nursing, for the purpose of preventing decline and to maintain the highest practicable level of functioning.
Responsibilities for the Wound Care are:
To Maintain Pressure Ulcer Wound Log, if size, location, admitted, acquired treatment. Monthly report due by the 5th of each month, • Visually inspect each wound weekly, obtain accurate measurements and complete dressing changes. Complete dressing changes on all Stage II-IV wounds on a daily basis depending on needs of facility. • Recommends treatment changes when improvement is not noted within 2 weeks • Consults DON, MD, Ostomy Nurse for difficult wounds, or those not responding to treatment • Recommends Specialty Beds or Overlays to treat wounds, notify Care Plan office • Maintains a log of residents using Specialty Beds and Serial Numbers. Keeps Nursing and Central Supply informed with start/stop dates for billing • Uses Treatment Cart room-to-room for dressing changes. Keeps treatment cart Stocked and clean. • Notifies Central Supply of dressing materials needed to maintain adequate supply for all shifts • Assures that all incontinent, immobile residents receive protective incontinence care each shift and an adequate supply of products are on hand. • Applies principles of Infection Control Techniques and if needed, Isolation for dressing changes. Monitors and completes in-service as needed on dressing changes, new wound care equipment and changes in orders for each resident • Responsible for completing wound rounds with MD and care plans • Assist with writing policies and procedures relating to wound care and infection control measures for residents • Maybe required to work assigned units if required when staffing problems occur • On Call rotation as scheduled • Ability to provide in-service on wound care and infection control as requested by the Administrator or Director of Nursing • Monitor for new antibiotic orders daily • Maintain infection control monitoring logs and flow sheets • Complete monthly infection surveillance report • Track and trend infection in facility • Culture equipment and areas of facility for bacteria overgrowth • Other Duties as assigned by the Director of Nursing or Assistant Director of Nursing
Qualifications for the Wound Care Nurse:
- Must be a Registered or Licensed Practical Nurse with license in good standing, with previous hospital, long-term care and restorative or wound care experience.
- Must be 18 years of age
- Must speak and understand English
- Must be able to supervise and instruct others.
- Previous supervisory experience or completion of appropriate training program, or as required by state law.
- Positive attitude toward the elderly.
