Avant Home Care is seeking a dependable and professional Licensed Practical Nurse (LPN) to perform in-home nursing visits for our clients throughout the Atlanta and North Georgia area, with an emphasis on Gwinnett and Hall counties.
This is an excellent opportunity for an LPN who enjoys working independently, providing one-on-one care, and traveling within the community.
Responsibilities
The LPN will conduct scheduled in-home visits and assist Avant Home Care with nursing and client-care responsibilities, including:
- Conducting scheduled client home visits
- Completing required nursing documentation accurately and on time
- Monitoring client condition and reporting changes or concerns
- Reviewing client care needs and services being provided
- Communicating with caregivers, clients, families, and Avant Home Care staff as appropriate
- Assisting with supervisory and other required nursing visits
- Ensuring documentation meets agency and applicable Georgia program requirements
- Reporting safety, compliance, or quality-of-care concerns promptly
- Working closely with Avant Home Care's RN and Client Operations team
Qualifications
Applicants must have:
- Current, active Georgia LPN license in good standing
- Valid driver's license and reliable transportation
- Ability and willingness to travel throughout assigned service areas
- Strong documentation and communication skills
- Ability to work independently and manage scheduled visits
- Professional and compassionate approach when working with clients and families
- Current CPR certification
- Ability to meet Avant Home Care's employment and compliance requirements
Home care, home health, Medicaid waiver, CCSP/SOURCE, or community-based nursing experience is preferred, but not required.
Service Area
Visits may be assigned throughout:
Greater Atlanta/Metro • Gwinnett County • Hall County • North Georgia
We will make every effort to coordinate visits geographically to minimize unnecessary travel.
Schedule
This is a flexible field-based position. Visits are scheduled based on client needs and regulatory requirements. This opportunity may be ideal for an LPN seeking additional income alongside another nursing position or a flexible nursing schedule.
