Avant-Guarde Healthcare provides consulting services to Skilled Nursing Facilities in Missouri and Arizona to assist clients in improving quality operations. We are currently searching for a seasoned CNA and CMT to join our team.

Our facility, Mountain View Manor, is located in Prescott, AZ and is a 118 licensed bed skilled nursing home. We are currently growing and searching for Team Oriented candidates to join us.
Responsibilities:
- Provide direct care to individuals with developmental disabilities in a home care setting
- Assist with activities of daily living, including bathing, dressing, and grooming
- Administer medications and treatments as directed by healthcare professionals
- Monitor and record vital signs, such as blood pressure and temperature
- Assist with medical documentation using electronic health record systems like Epic
- Assist with mobility and transfers, ensuring the safety and comfort of individuals
- Provide emotional support and companionship to individuals in their daily activities
- Collaborate with a multidisciplinary team to develop and implement care plans
Skills:
- Knowledge of human anatomy and physiology to understand the needs of individuals
- Familiarity with medical terminology and documentation practices
- Strong communication skills to effectively interact with individuals, families, and healthcare professionals
- Compassion and empathy towards individuals with developmental disabilities
- Ability to work independently and as part of a team in a fast-paced environment
- Attention to detail and ability to follow instructions accurately
