Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including long-term care, assisted living, and acute care environments. As an LPN, you will work under the supervision of registered nurses and physicians to deliver essential nursing services while ensuring adherence to best practices and regulatory standards.
Responsibilities
- Provide direct patient care, including administering medications and treatments as prescribed.
- Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
- Assist with daily living activities, including bathing, dressing, and feeding patients.
- Maintain accurate medical documentation and records in compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Educate patients and their families on health management and disease prevention strategies.
- Perform wound care, catheterization, IV insertion, and other clinical procedures as required.
- Support patients with developmental disabilities, dementia care, Alzheimer's care, and other specialized needs.
- Participate in discharge planning and case management activities to facilitate smooth transitions of care.
Skills
The successful candidate should possess a range of skills that enhance their ability to provide excellent patient care. Preferred skills include:
- Experience with Epic EMR systems, Cerner, or Athenahealth for efficient medical documentation.
- Critical care experience in Level I or Level II trauma centers or ICU settings.
- Knowledge of ICD-9 and ICD-10 coding for accurate medical billing.
- Proficiency in phlebotomy, venipuncture, and specimen collection/processing.
- Familiarity with pain management techniques and acute pain management protocols.
- Ability to perform physical examinations and diagnostic evaluations effectively.
- Strong understanding of anatomy, physiology, medical terminology, and infection control practices.
- Experience in home care or community health settings is a plus.
- Competence in administering injections, tube feeding, ventilator management, and airway management.
