Avail Health is hiring a full-time Mobile Nurse Practitioner to deliver comprehensive care to medically complex seniors in skilled nursing facilities. Own your panel. Practice at the top of your license. Autonomous, field-based work — Monday through Friday.
What You'll Do
- Manage a dedicated SNF patient panel with routine and acute visits
- Assess, diagnose, and treat chronic conditions and geriatric syndromes
- Lead care transitions — admissions, readmissions, and discharges
- Conduct Health Risk Assessments and build individualized care plans
- Collaborate with RNs, care coordinators, and facility staff
Compensation & Perks
- $135,000–$155,000 base salary DOE

Requirements
- MSN or DNP; active Maryland NP license in good standing
- FNP or Adult-Gerontology NP certification
- Maryland DEA with prescriptive authority (or eligible before start)
- 1–3 years clinical experience; SNF, LTC, or geriatrics preferred
- Valid MD driver's license, reliable transportation, and auto insurance

Pay: $135,000.00 - $155,000.00 per year
