Pulmonary Program Coordinator (RN)
Salary: $72,800- $83,200 annually (depending on experience)
Hourly Equivalent: $35- $40
Shift: Full-Time
Autumn View Health Care Facility is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate our Pulmonary Program. In this role, you'll provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs.

RESPONSIBILITIES:
- Oversee and coordinate daily operations of the Pulmonary Program
- Coordinate individualized care plans for residents with pulmonary and respiratory needs
- Monitor residents with respiratory and pulmonary conditions
- Collaborate with physicians, respiratory providers, therapy, nursing, and interdisciplinary teams
- Assist with admissions, assessments, and care coordination for pulmonary residents
- Provide staff education on pulmonary care and clinical best practices
- Ensure compliance with regulatory and clinical standards
- Participate in quality improvement and program development initiatives
REQUIREMENTS:
- Active New York State Registered Nurse (RN) license required
- Skilled nursing, rehabilitation, pulmonary, respiratory, or subacute experience preferred
- Strong clinical assessment and care coordination skills
- Experience with care planning and interdisciplinary collaboration preferred
- Strong leadership, organizational, and communication skills
