Occupational Therapist - Home Health

AuthoraCare Collective is currently seeking an Occupational Therapist for our Home Health department supporting our Forsyth, Davie, Stokes and Northern Davidson counties service areas. This is a full-time position, Monday - Friday 8:00am - 5:00pm.


Must be able to travel to, enter, and function in the homes of any patient within the service area.


The Occupational Therapist provides services to restore function, improve mobility, relieve pain, prevent, or limit permanent physical disabilities of patients suffering from injuries and disease. Manage the therapy Plan of Treatment for a case load of Integrated Health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient problems, establish therapy goals, and interventions.


Work with physician to establish a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease pain and help increase mobility and endurance. Establish and maintain therapeutic relationship with patient, including providing community support for resources and education. Therapist may evaluate and treat hospice patients as needed. Document the goals and outcomes of care. Education and Experience
:

  • Degree from an accredited Occupational Therapy Program
  • A minimum of six months of occupational therapy experience preferred
  • Home Health experience a plus
  • Current and unrestricted OT licensure
  • Current CPR certification

Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.


Key Responsibilities:

Delivery of Patient Care

  • Directs and coordinates the care and services provided to patients in accordance with the patient’s Plan of Treatment based on all assessments.
  • Responsible for continuous assessment of patient/family needs and implementation of interdisciplinary Plan of Treatment.
  • Works with IDT to identify patient/family goals of care and establish and maintain the Plan of Care.
  • Performs regular re-evaluation of the patient/family/caregiver situation to meet their needs and maintain continuity of care.
  • Recognizes influence of culture, ethics, and beliefs on clinical decision making.
  • Develop and provide copies of home exercise plan to patient.
  • Makes home visits in accordance with Plan of Treatment.
  • Communicates changes to the Plan of Care to patient/facility as applicable.
  • Delegates care for the Occupational Therapy assistant in accordance with state regulatory requirements providing indirect supervision to Occupational therapy assistant.
  • Complies with infection control and exposure control policies.
  • Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives. Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient's level of function.
  • Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Treatment.
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion, and function or teach compensation measures.
  • Review/expand the retraining programs in a manner consistent with the behavioral goals of each patient and within the guidelines of the rehabilitation program.
  • Consult with other vocational team members, as appropriate, to develop and implement vocationally oriented plans consistent with the needs and capabilities of patients.
  • Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient, and family members.
  • Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's needs.
  • Design community reintegration activities to assist the client in the physical reconditioning effort, and/or the psychological adjustment and coordinate the plan with members of the interdisciplinary team.
  • Recommend and/or design special adaptive equipment for clients to improve residence or working environments or improve their participation in the rehabilitation program and/or community.
  • Design/train staff and family members to carry out the retraining program including dressing, feeding, grooming and hygiene skills, participate in team and family educational meetings.
  • Accurately and thoroughly document patients' care observations, interventions and evaluation on the day services are rendered. Ensure that interim (verbal) orders received from the physician are accurately documented and implemented. Submit evaluation, treatment plans and discharge summary to the supervisor and care management staff.
