What are the responsibilities and job description for the Military Case Manager 1 (Full-Time) position at Aurora San Diego?
Position Summary: To provide direct and indirect patient care and case management services to active-duty military patients receiving inpatient care for substance abuse, mood disorders and/or posttraumatic stress disorder.
*Pay Range: $22/hr. to $25/hr.
Essential Duties:
- Manage multiple cases from admit to discharge
- Command and Provider communication
- Work within a multidisciplinary clinical team
- Write brief reports and clinical updates
- Take care of patient needs during their treatment
- Discharge planning
- Attend clinical treatment teams
- Administrative: faxing, filing, report preparation
Education:
- Bachelor's degree in Psychology or Social Work or related clinical field
Experience:
- A minimum of one year work experience pre or post-graduation
- Interest in the field of Behavioral Health
- Strong oral and written communication skills
- Ability to work effectively both independently and as part of an interdisciplinary treatment team
Desired Qualifications:
- Professional experience working with military families or personal military lifestyle experience
- Knowledge of the difficulties of dealing with the requirements of military life
- Knowledge of military and community resources appropriate for patients
