Family Nurse Practitioner (FNP) – Woodstock, IL

SUMMARY

Provide general medical care and treatment to patients under the direction of a Physician, following established policies, protocols, and evidence-based practice guidelines.

ESSENTIAL DUTIES & RESPONSIBILITIES

• Provide routine primary care, treatment, family planning, and prenatal services as requested.

• Order, interpret, and evaluate diagnostic tests to assess patient needs.

• Prescribe medications as guided by policies and procedures.

• Refer patients to Physicians or specialized resources as appropriate.

• Participate in quality improvement initiatives and complete required training.

• Perform other related duties as assigned.

QUALIFICATIONS

• Master’s degree from an accredited school of nursing and Nurse Practitioner Program.

• Current APRN licensure in Illinois with FNP certification.

• Current BLS certification.

• Valid driver’s license and auto insurance; ability to travel to all Aunt Martha’s locations.

• Availability for occasional evening or weekend emergencies.

• Bilingual preferred.
