Family Nurse Practitioner (FNP) – Joliet/Jefferson

SUMMARY
Provide general medical care and treatment to patients under the direction of a Physician, following established policies, protocols, and evidence-based practice guidelines.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provide routine primary care, treatment, family planning, and prenatal services as requested.
Order, interpret, and evaluate diagnostic tests to assess patient needs.
Prescribe medications as guided by policies and procedures.
Refer patients to Physicians or specialized resources as appropriate.
Participate in quality improvement initiatives and complete required training.
Perform other related duties as assigned.
QUALIFICATIONS
Master’s degree from an accredited school of nursing and Nurse Practitioner Program.
Current APRN licensure in Illinois with FNP certification.
Current BLS certification.
Valid driver’s license and auto insurance; ability to travel to all Aunt Martha’s locations.
Availability for occasional evening or weekend emergencies.
Bilingual preferred.
