Family Medicine, Internal Medicine, and IM PEDS
Summary
This position aims to provide high-quality patient care and ensure effective clinic operations. Patient care includes providing comprehensive medical services for members of family. Services include but are not limited to: preventive care, immunizations, primary care and treatment, health education, prescribing of medications, ordering and interpreting lab testing, physical examinations, STD screenings, vision/hearing, women’s health, family planning, prenatal services, nutrition. Provider may be used in various locations depending on the organization's needs. Clinic shifts include 1 evening a week and 1-2 Saturdays/month. This position may also include supervising physician assistants and/or nurse practitioners and training resident physicians or other health provider interns/externs.

EDUCATION And/or EXPERIENCE
Graduate of an accredited medical school with a degree of Doctor of Medicine (MD) or Doctor of Osteopathy (DO); completion of an Internship and an Accredited Residency Program required.

REQUIRED EDUCATION, EXPERIENCE And COMPETENCIES
- Provide continuous, comprehensive primary care for infants, children, youth, and adults according to current standards of care.
- Take patient histories.
- Perform or request diagnostic tests.
- Make diagnoses and prescribe medication as necessary.
- Administer vaccinations and perform follow-up examinations.
- Educate patients in disease prevention and health maintenance.
- Prepare all medical records, clinical reports, and correspondence.
- Support quality initiatives.
- Work in collaboration with the management team and providers.
- Complete all Core Curriculum training(s) required for the position.
- Perform all other duties as assigned.
- Active Illinois State Medical License, DEA certificate
- Board Certified or Board Eligible based on governing board guidelines
- Providers are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when management requests.
- Experience should be aligned within scope of training.
- Familiarity with Electronic Health Records (EHR) is preferred.
