Job Summary
We are seeking a compassionate and dedicated Wraparound Facilitator to coordinate and support individualized care plans for youth and their families. The ideal candidate will serve as a key liaison among clients, families, and community service providers, ensuring comprehensive, culturally sensitive, and effective support. This role requires strong interpersonal skills, a commitment to client-centered approaches, and knowledge of mental health and social services. Experience or training in art therapy, psychopharmacology, or gender issues counseling is highly desirable to enhance the quality of care provided.
Duties
- Facilitate wraparound planning meetings to develop personalized service plans that meet the unique needs of youth and families
- Coordinate with multidisciplinary teams including mental health professionals, schools, and community agencies to ensure seamless service delivery
- Support families in navigating available resources and advocating for their needs
- Monitor progress toward goals outlined in the care plan and adjust strategies as necessary
- Provide emotional support and guidance to clients during challenging situations
- Document all interactions accurately in compliance with agency policies and confidentiality standards
- Promote strengths-based, family-driven approaches that empower clients and their support systems
- Incorporate art therapy techniques when appropriate to foster expression and healing
- Address gender issues counseling as part of individualized plans to ensure sensitive and inclusive support
- Stay informed about developments in mental health treatment, psychopharmacology, and social services
Requirements
- Bachelor’s degree in social work, psychology, counseling, or a related field; relevant experience may substitute for education
- Strong understanding of mental health issues, family dynamics, and community resources
- Excellent communication and facilitation skills with diverse populations
- Ability to work collaboratively within multidisciplinary teams
- Knowledge or training in art therapy techniques is preferred
- Familiarity with psychopharmacology principles is advantageous
- Experience or training in gender issues counseling is a plus
- Ability to manage multiple cases effectively while maintaining attention to detail and confidentiality
