Job Summary
Augusta Levy Behavioral Services is seeking a dedicated and family-centered CSED Waiver Wraparound Facilitator to support children and youth with complex behavioral health needs and their families. This role coordinates the Wraparound process in alignment with CSED Waiver requirements, emphasizing strengths-based planning, collaboration, and individualized care.
Key Responsibilities
- Facilitate the Wraparound process in compliance with CSED Waiver and Wraparound fidelity standards
- Engage children, youth, and families using a strengths-based, culturally responsive approach
- Coordinate and lead Child and Family Team (CFT) meetings
- Develop, implement, and monitor individualized service plans
- Collaborate with care managers, service providers, schools, and community partners
- Assist families in accessing waiver services, natural supports, and community resources
- Maintain timely and accurate documentation in accordance with CSED Waiver and agency requirements
- Monitor service effectiveness and adjust plans as needed to support youth and family goals
Job Type: Full-time
Pay: $35,000.00 - $41,000.00 per year
