Overview
We are seeking a dedicated Memory Care Coordinator to join our team in providing exceptional care to our residents. This position will require working one weekend each month. The ideal candidate will have experience as a certified medication aide or LPN in long term care or hospice. This role involves overseeing the care of individuals with cognitive impairment. As a Memory Care Coordinator, you will play a crucial role in enhancing the quality of life for our residents through effective leadership and coordination of care services.
Responsibilities
- Coordinate and oversee daily activities within the community, ensuring compliance with all regulations and standards.
- Develop and implement individualized care plans for residents, focusing on their unique needs and preferences.
- Supervise, educate, and mentor staff members, providing guidance and support to ensure high-quality care delivery.
- Conduct regular assessments of resident needs and progress, adjusting care plans as necessary.
- Facilitate communication between residents, families, and healthcare providers to promote comprehensive care.
- Administer medications as required and ensure proper documentation is maintained.
- Lead training sessions for staff on best practices in memory care, long-term care, and working with individuals with cognitive impairment.
- Maintain a safe and supportive environment for all residents.
Requirements
- Previous experience in senior care settings such as long term care and hospice is preferred.
- Strong leadership skills with the ability to supervise and motivate staff effectively.
- Knowledge of medication administration protocols and best practices in resident care.
- Experience working with individuals with dementia and cognitive impairment is highly desirable.
- Excellent communication skills to interact effectively with residents, families, and team members.
