Responsibilities
- Conduct comprehensive patient assessments to determine eligibility for hospice care, including evaluating physiological status and medical history.
- Manage the intake process by coordinating with healthcare providers, families, and interdisciplinary teams to facilitate smooth admissions.
- Collaborate with physicians, social workers, and other healthcare professionals to develop individualized care plans tailored to each patient's needs.
- Ensure accurate documentation of patient information within Electronic Health Records (EHR) systems while maintaining strict compliance with HIPAA regulations.
- Provide education and support to patients and families regarding hospice services, care options, and discharge planning.
- Monitor patient status during admission and adjust care plans as necessary to optimize comfort and quality of life.
- Participate in ongoing case management activities, including admits, routine and discharge planning and post-admission follow-up to ensure continuous patient-centered care, as well as death visits.
Qualifications
- Proven critical care experience, including ICU or emergency medicine settings, demonstrating strong clinical assessment skills.
- Experience working in Hospice care, not required
- Knowledge of infectious disease care, behavioral health considerations, geriatrics, and post-acute or outpatient care settings.
- Familiarity with hospital experience including discharge planning, utilization management, and managing patients as a nurse case manager.
- Proficiency in Electronic Health Records (EHR) systems and understanding of HIPAA regulations for patient data security.
- Excellent communication skills for client interaction, interdisciplinary collaboration, and patient service delivery.
- Ability to perform vital signs assessment, diagnostic evaluation, medical terminology comprehension, and clinical case management effectively.
- Strong organizational skills with attention to detail in managing medical records and coordinating intake processes.
- Clear RN license
