At Auburn Crest Primary & Palliative Care, we bring high-quality medical care directly to patients where they live; in private homes, assisted living facilities, and memory care communities. As a Medical Assistant on our mobile care team, you'll travel alongside providers throughout the designated area, delivering hands-on clinical support to patients. This role is for someone who thrives outside the traditional clinic setting, is organized, compassionate, and comfortable working independently in the community.

What You'll Do

Clinical Support
  • Travel with providers to patient homes and care facilities, preparing for each visit with organized supplies, equipment, and patient schedules
  • Obtain and document vital signs, medical history updates, and chief complaints in the home setting
  • Assist providers during in-home examinations and procedures
  • Perform clinical tasks within scope
  • Observe and promptly report changes in patient condition to the provider

Documentation & Care Coordination
  • Document patient information accurately in the EMR in real time or shortly after visits
  • Coordinate care with hospice agencies, home health, pharmacies, labs, and DME providers
  • Assist with scheduling follow-up visits and daily route logistics
  • Facilitate communication between patients, families, and the care team
  • Schedule annual Medicare Wellness Visits

Patient & Family Support
  • Provide compassionate, dignified care in diverse home environments
  • Educate patients and caregivers on medications, care plans, and provider instructions
  • Support patients and families navigating chronic illness and end-of-life transitions
  • Assist with advance directives and hospice referrals when appropriate

Equipment & Compliance
  • Maintain, clean, and restock mobile medical equipment and supply kits
  • Follow infection control, safety, and HIPAA protocols across all care settings
  • Comply with all organizational policies and regulatory requirements

What We're Looking For
  • High school diploma or equivalent
  • Completion of an accredited Medical Assistant program or equivalent experience
  • Current CPR/BLS certification
  • Valid driver's license, reliable transportation, and auto insurance
  • Ability to travel throughout the Coeur d'Alene service area
  • Strong organizational, communication, and time-management skills
  • Ability to work independently in non-clinical environments

Preferred
  • Experience in home-based care, primary care, geriatrics, hospice, or palliative care
  • Phlebotomy experience
  • Familiarity with EMR systems and mobile documentation tools
