Position Summary
We are looking for a compassionate, community-oriented individual to join our team as a Community Health Navigator. In this role, you will help individuals and families in Cook County connect to essential healthcare services, including support with Medicaid enrollment and renewals.
You do not need prior experience with Medicaid redetermination — we provide comprehensive training and ongoing support. What matters most is your ability to connect with people, communicate clearly, and care about improving access to healthcare in your community.
This contractor role is ideal for someone who:
- Is bilingual in Spanish and English
- Enjoys working directly with community members
- Is dependable and timely
- Is comfortable engaging with people in public and community settings
- Is passionate about helping others access healthcare resources
What You’ll Do
- Support community members at local events, health fairs, and partner sites
- Provide one-on-one guidance in a welcoming and respectful manner
- Help individuals understand their healthcare coverage options
- Connect residents to Cook County Health services and resources
- Participate in training and team meetings to build your knowledge and skills
What We’re Looking For
- Fluency in Spanish and English (strongly preferred)
- Friendly, approachable, and culturally responsive communication skills
- Ability to work independently in community settings
- Commitment to health equity and serving diverse populations
- Experience in community outreach, customer service, healthcare, or social services is helpful — but not required
