Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs and reporting changes to healthcare providers
- Administer medications and manage medical documentation in compliance with HIPAA regulations
- Support patients with memory care needs such as Alzheimer’s disease and dementia through specialized techniques and compassionate engagement
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively
- Maintain a clean and safe environment by performing light housekeeping tasks like laundry, cleaning, and meal preparation
- Document patient care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
Experience
- Prior experience in home health care or assisted living environments preferred
- Experience working with individuals with developmental disabilities or dementia care is highly valued
- Knowledge of medical terminology, medication administration, and vital signs monitoring is essential
- Familiarity with hospice care protocols and long-term care settings enhances your ability to provide comprehensive support
- Certification in CPR and first aid required; additional certifications such as hospice or Alzheimer’s care are advantageous
- Proven ability to work independently while following detailed care plans and medical instructions
- Strong communication skills to collaborate effectively with healthcare teams, patients, and families
