Registered Nurse (RN)- PRN

Employment Type: PRN

Compensation: Competitive Pay (starting at $75 per hour)

Location: Colorado Springs and Pueblo

• Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
• Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
• Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
• Collaborate with interdisciplinary team members to ensure coordinated, high-quality care
• Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards
• Supervise and delegate care to Home Health Aides to ensure quality and adherence to the plan of care
• Available to drive and travel within Colorado springs and Pueblo for different service plans depending on location of clients. (Mileage is paid out at the Federal mileage rate)

• Active Registered Nurse (RN) license in the state of practice
• Experience in home health, or a related healthcare setting preferred
• Strong clinical assessment and critical thinking skills
• Knowledge of home health regulations and documentation standards
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
