Requirements:
- Active Registered Nurse License in the state of NJ – We accept Compact Licenses (must reside in compact license state)
- Tuberculosis (TB) screening/QuantiFERON/Chest X-Ray (within 1 year)
- Proof of U.S. work authorization (valid, unexpired documents: government issued ID)
- Reliable transportation
Responsibilities:
- Conducting Start of Care (SOC) visits to evaluate medical, physical, and emotional needs and creating individualized care plans.
- Performing regular reassessments to monitor progress and adjust care plans as needed.
- Providing patient and family education on condition management, medication adherence, and preventive health measures.
- Collaborating with physicians, therapists, social workers, and families to ensure coordinated, comprehensive care.
- Maintaining accurate clinical documentation in compliance with New Jersey Board of Nursing regulations and agency policies.
- Supervising and supporting Certified Home Health Aides (CHHAs) and other care team members.
