Responsibilities
- Deliver comprehensive nursing care by performing physical examinations, vital signs assessment, and patient evaluations with precision and compassion.
- Administer medications and injections accurately.
- Assist with medical procedures such as catheterization, suturing, tube feeding, and sterile processing to support patient recovery and comfort.
- Utilize electronic health record (EHR) systems like Point Click Care to document patient information thoroughly and maintain compliance with HIPAA regulations.
- Conduct patient triage in urgent care or emergency settings, prioritizing care based on acuity levels and critical needs.
- Collaborate with multidisciplinary teams on discharge planning, case management, and coordination of post-acute or home & community care services.
- Support infection control protocols and maintain aseptic techniques to ensure a safe environment for patients and staff.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
- Proven experience in diverse clinical settings such as hospital medicine, long-term care, hospice, or emergency rooms.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation procedures.
- Experience working with individuals with developmental disabilities or dementia-related conditions like Alzheimer’s or memory care needs.
- Proficiency in EMR/EHR systems such as Point Click Care for accurate medical documentation.
- Ability to perform complex patient assessments including vital signs measurement, physical examinations, specimen collection/processing, and venipuncture.
- Excellent communication skills to work effectively with patients of all ages—including pediatrics and neonatal ventilator management—and collaborate within multidisciplinary teams.
