Who we are: Attain Behavioral Health a growing behavioral health provider serving Cleveland and Toledo. We deliver TBS and other services in collaboration with nursing home facilities to support residents’ behavioral and emotional needs. We are hiring 5 clinicians to expand our TBS program.

Position summary: As a TBS Group Facilitator you will run evidence-informed therapeutic groups inside nursing homes, complete individual assessments and short interventions, coordinate with facility interdisciplinary teams, and maintain compliant documentation for billing.

Key duties

• Prepare and deliver structured TBS group curriculum tailored to nursing home populations.
• Complete assessments, treatment planning, and individual brief sessions as needed.
• Maintain timely and accurate progress notes, billing records, and outcomes tracking.
• Communicate with facility staff, families, and Attain leadership about resident progress.
• Attend periodic staff training and supervision; participate in quality improvement.
  
  
  

Required

• QMHS-3 credential OR Ohio Licensed Social Worker (LSW).
• Valid driver’s license and reliable vehicle.
• Background check and ability to work in long-term care settings.
  
  
  

Preferred

• Experience with geriatric behavioral health, dementia care, or SUD in older adults. Group facilitation experience.

Shift: 9am-5pm full-time, per-diem. Looking for candidates who can start immediately, offering $500 sign on bonus after 90 days of employment.
