As a Wound Care Nurse (RN/LPN), you'll play an important role in protecting residents' skin integrity, preventing pressure injuries, and supporting healing through individualized care and clinical expertise.

What You'll Do
- Assess, monitor, and document wounds to evaluate healing progress, identify changes, and determine appropriate interventions
- Develop, implement, and update individualized wound care plans based on resident needs and provider orders
- Perform wound treatments, dressing changes, and other prescribed interventions in accordance with clinical standards
- Identify residents at risk for pressure injuries and skin breakdown and implement prevention strategies
- Monitor wound healing and recognize signs of infection or complications, promptly communicating concerns to the appropriate healthcare providers
- Collaborate with the Director of Nursing, providers, nursing team members, and interdisciplinary team to coordinate wound care and improve resident outcomes
- Educate nursing team members on wound prevention, treatment techniques, skin assessments, and documentation standards
- Ensure accurate and timely wound assessments, treatment records, and clinical documentation
- Monitor the effectiveness of wound care interventions and recommend changes when needed
- Support compliance with state and federal regulations, infection prevention standards, and clinical best practices
- Administer medications and provide other nursing treatments as needed
- Supervise and support nursing assistants in providing safe, compassionate resident care
What You Bring
- Current Registered Nurse (RN)/ Licensed Practical Nurse (LPN) license in good standing
- Experience in wound care, skilled nursing, or long-term care preferred
- Wound care certification preferred
- Strong knowledge of wound assessment, treatment, pressure injury prevention, and skin integrity management
- Ability to recognize changes in resident condition and respond appropriately
- Knowledge of infection prevention, clinical documentation, and applicable state and federal regulations
- Ability to develop and implement individualized care plans and evaluate treatment outcomes
- Strong communication, organization, and clinical judgment
- Ability to educate and support nursing team members in delivering quality care
- Medication administration and IV certification as required for the position
