Since social workers are 32-hours per week, one social worker must be onsite at opening (8:30 a.m. Monday-Thursday and 8 a.m. Friday) and the other comes in later and closes at 4:30 Monday - Thursday and 4 p.m. Friday. Currently, the social worker on staff does the opening shift.
Pay Range
$26.55 - $39.85
Major Responsibilities:
Education
Bachelor’s or master's degree in social work (MSW) from a Council on Social Work Education (CSWE)- accredited program preferred.
Certification / Registration / License
Licensed Masters or Bachelor of Social Work or Licensed Clinical Social Work issued in the state in which the teammate works
If unlicensed the social worker will obtain license within 6 months of hire date.
Experience
Social work and/or medical case management experience in a healthcare setting is desirable.
Knowledge / Skills / Abilities
Demonstrates knowledge of social work principles and methods and the ability to apply these in a health care setting.
Demonstrates time management, professional accountability, and documentation skills.
Demonstrates communication skills and the ability to collaborate within a multi-disciplinary team.
Demonstrates and acts in accordance with safety principles of an accountable care organization.
Physical Requirements And Working Conditions
Position requires sitting, standing, speaking, reading/writing, and walking throughout the workday.
Exposed to patient care environment. May be exposed to hazardous materials and life-threatening diseases, therefore team members must abide by personal protective equipment as ordered.
Operates all equipment necessary to perform the job.
Exposed to a normal office environment.

The Service Navigator plays a vital role in delivering compassionate, patient-centered care within Cancer Navigators. This position provides comprehensive social work services, medical case management, and care coordination to individuals living with cancer and their families. Working closely with the patients’ medical team, outside resources and external partners to help the patient navigate complex medical and social systems, reduce barriers to care, and achieve improved health outcomes across the continuum of care.

Major Responsibilities:
- Conducts comprehensive biopsychosocial assessments by interviewing patients and families and evaluating social determinants of health including financial, emotional and social needs.
- Implements interventions substantiated by assessment as appropriate to the needs of the patient/client system and consistent with available resources.
- Develops and maintains an accurate list of community and national resources and refers patients based on their needs.
- Develop, implement, and regularly reassess individualized care needs of patients and families and make referrals for patients and their families who require stress management or further mental health assessments.
- Provide continuous monitoring of patient progress toward completion or maintenance of treatment plan and adjust interventions as needed, documenting all patient/family encounters and updates in a timely manner.
- Provides compassionate emotional support to patients and their families and is available to facilitate groups.
- Assist patients in completing paperwork to apply for resources, including but not limited to, social security, Women’s Health Medicaid, Veterans benefits, and national foundations.
- Adheres to all HIPPAA guidelines and policies and procedures.
- Collaborates with nurses, physicians, and other providers to obtain the best outcome for each patient and responds to referrals within 48 hours.
- Maintains professional standards and responsibilities for his/her own professional practice according to accreditation, hospital, system, state and NASW Standards and Code of Ethics. Completes all required continuing education to maintain licensure and increase knowledge within area of practice specialty.
