What You’ll Do
- Assist the Social Services Director in implementing and carrying out social service programs in accordance with state and federal guidelines
- Support the identification and documentation of residents’ social, emotional, and psychological needs
- Provide service coordination and support for residents and families, including communication and follow-up
- Participate in care planning and interdisciplinary team meetings as assigned
- Maintain accurate, timely documentation to support compliance and regulatory standards
- Associate’s Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree)
- Experience working in a skilled nursing or long-term care setting preferred
- Strong written and verbal communication skills, with the ability to interact professionally with residents, families, and interdisciplinary teams
- Ability to assist with documentation, assessments, care planning, and regulatory requirements under the direction of a licensed Social Services Director
- Compassionate, resident-focused approach with strong interpersonal skills
- Organized and detail-oriented, with the ability to manage multiple priorities
- Working knowledge of resident rights, discharge planning, and psychosocial support services preferred
- Positive, can-do attitude with excellent attendance
