You’ll work closely with residents, families, and interdisciplinary teams to ensure emotional, psychosocial, and social needs are met with dignity and respect.
What You’ll Do
- Develop, plan, implement, and evaluate social service programs in accordance with state and federal guidelines
- Identify and address residents’ social, emotional, and psychological needs
- Provide service coordination for residents and families
- Support care planning and interdisciplinary collaboration
- Ensure compliance with applicable regulations and documentation standards
- Licensed Social Worker preferred, but not required
- At least one year of supervised social work experience in a healthcare setting (elderly psychosocial experience preferred)
- Working knowledge of nursing home regulations, including MDS procedures
- Familiarity with Medicare and Medicaid reimbursement and eligibility guidelines
- Strong written and verbal communication skills
- Positive, can-do attitude with excellent attendance
