Responsibilities:

• Develop, plan, implement and evaluate service program and service in accordance with state and federal guidelines.
• Identify and provide for resident's social, emotional and psychological needs
• Provide service coordination
  
  
  

QUALIFICATIONS:

• Licensed Social Worker, preferred – not required
• One year of supervised social work experience in a healthcare setting, preferably with the psychosocial of the elderly
• Working knowledge of nursing homes regulations, including MDS procedures, and reimbursement and eligibility guidelines under Medicare and Medicaid
• Excellent communication skills; written and verbal
• Positive Can-Do Attitude
