Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or assisting with transfers.
- Monitor patient health by taking vital signs, observing for changes in condition, and reporting findings promptly to healthcare professionals.
- Prepare nutritious meals tailored to dietary restrictions and preferences, ensuring proper meal planning and safe food handling practices.
- Administer medications accurately following care plans while maintaining compliance with HIPAA regulations to protect patient privacy.
- Support clients in engaging activities that promote social interaction, mental stimulation, and overall well-being within home or community settings.
- Maintain detailed documentation of care provided, including medication administration, patient observations, and updates to care plans.
- Perform light cleaning duties to ensure a hygienic environment that promotes health and safety for residents or clients.
Qualifications
- Proven experience in home health care, assisted living facilities, nursing homes, or group homes working with seniors or individuals with disabilities.
- Knowledge of caregiving best practices including Alzheimer’s care, dementia support, autism support, and hospice care.
- Certifications such as CPR (Cardiopulmonary Resuscitation), First Aid, and training in HIPAA compliance are highly desirable.
- Ability to lift heavy objects safely and perform physical tasks required for patient transfers and mobility assistance.
- Strong observational skills for patient monitoring and recognizing signs of distress or deterioration.
- Excellent communication skills to collaborate effectively with healthcare teams, patients, families, and social workers.
