Atlas Homecare & Hospice is seeking a compassionate, organized, and clinically skilled Registered Nurse Case Manager to join our growing team. This position is responsible for providing comprehensive nursing assessments, coordinating patient care, developing individualized plans of care, and ensuring the highest quality of service for patients in their homes.
The ideal candidate is an experienced Registered Nurse who thrives working independently while collaborating with an interdisciplinary team to deliver exceptional patient-centered care. This position will initially provide coverage throughout St. George and Cedar City, with territory assignments adjusted as patient census and organizational growth support the addition of future nursing staff.
Responsibilities
- Complete comprehensive nursing assessments and ongoing evaluations of patient needs.
- Develop, implement, and update individualized plans of care based on physician orders, clinical assessments, and patient goals.
- Manage a caseload of home health and/or hospice patients while ensuring quality, continuity, and timely delivery of care.
- Provide case management services throughout the St. George and Cedar City service areas. Initial coverage will include both markets, with territories adjusted as census and staffing needs evolve.
- Monitor patient conditions and respond appropriately to changes in clinical status.
- Coordinate care with physicians, therapists, social workers, home health aides, and other members of the interdisciplinary team.
- Educate patients and family members regarding diagnoses, medications, disease management, safety, and available community resources.
- Advocate for patients while promoting independence, dignity, and quality of life.
- Maintain accurate, timely, and compliant electronic documentation.
- Participate in on-call rotation as assigned.
- Ensure compliance with Medicare Conditions of Participation, state regulations, and agency policies.
- Build positive relationships with patients, families, referral sources, and community partners while representing Atlas Homecare & Hospice with professionalism.
Qualifications
- Current, unrestricted Registered Nurse (RN) license in the State of Utah.
- Graduate of an accredited school of nursing.
- Minimum of one (1) year of nursing experience required.
- Home Health, Hospice, Emergency Department, ICU, Medical-Surgical, or Case Management experience preferred.
- Current BLS certification.
- Strong clinical assessment, critical thinking, and decision-making skills.
- Excellent communication, documentation, and organizational abilities.
- Ability to work independently while effectively managing multiple priorities.
- Comfortable using electronic medical records (EMR) and other healthcare technology.
- Valid driver's license, reliable transportation, and proof of current automobile insurance.
