Role Description
This is a full-time, on-site Social Worker position located in Cranford, NJ. The Social Worker in Long-Term Care will provide comprehensive social services to residents and their families while collaborating with interdisciplinary teams. Primary responsibilities include conducting assessments, creating care plans, providing emotional support, coordinating discharges, addressing psychosocial needs, and advocating for residents’ best interests. The role involves engaging with residents to enhance their quality of life and ensure their well-being.
Qualifications
- Proficient in conducting psycho-social assessments, crisis intervention, and care planning
- Skilled in providing counseling, emotional support, and advocacy for residents and their families
- Strong knowledge of long-term care regulations and discharge planning protocols
- Excellent interpersonal, communication, and relationship-building abilities
- Ability to work in a multidisciplinary team and collaborate with healthcare professionals
- Bachelor's or Master’s degree in Social Work or a related field (Master’s degree preferred)
- Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW) or Certified Social Worker (CSW) credentials in New Jersey
- Experience in long-term care, geriatric services, or healthcare settings is highly desirable
