Position Summary: This position will provide supportive services including coaching in skills related to consumer's needs to assist the consumer in maintaining community tenure. As part of the Comprehensive Wellness Center, this position assists consumers with community integration, goal setting and wellness planning.
Duties and Responsibilities
Education: High school diploma or GED. Associate's degree in behavioral health field preferred.
Experience: Peer position requires that the applicant self-declare past or current treatment for a mental illness. One year experience in a community based self-help service or behavioral health setting is preferred. Must have the appropriate Mental Health Association (MHA) training, or would complete such training within one year of employment.

Duties and Responsibilities
- Engages clients in a warm and empowering manner. Utilizes a first person approach to interactions and interventions with clients.
- Assesses social service needs of clients.
- Assists clients with linkage to services in the community, such as: financial assistance, medical assistance, transportation, etc.
- Coordinates clients' behavioral health and medical care within the Comprehensive Wellness Center program.
- Performs outreach and engagement of clients to increase compliance.
- Completes assessments for the Comprehensive Wellness Clinic and data collection for TRAC system.
- Participates in daily rounding meetings regarding clients.
- Participates in a weekly treatment team meeting that incorporates both the behavioral health and medical staff.
- Collects medical data needed for reporting and outcome measurements.
- Assists clients with scheduling of appointments for the behavioral health and medical care.
- Conducts wellness prevention groups to clients in the program.
- Participates with other team members in the completion of treatment plans for clients within established time frames.
- Collaborates with client's behavioral health care providers regarding treatment planning and discharge planning as indicated.
- Assists with maintaining compliance with DMHAS, Joint Commission, and SAMHSA contract requirements.
- Completes documentation within the electronic medical record systems and this documentation is completed in a timely manner.
