Hospice Registered Nurse - Evening Shift

Schedule

This role would be for the evening hours 3 PM - 11 PM and would require every other weekend commitment.

Job Description

We are seeking an experienced Hospice Registered Nurse to join our team. This position involves caring for patients in hospice, ensuring their comfort and dignity at end-of-life stages. The ideal candidate will have a strong background in home hospice care and be able to provide medication recommendations for managing end-of-life symptoms.

Responsibilities

• Perform patient assessments and develop care plans based on patient and family needs.
• Maintain visitor productivity standards and demonstrate knowledge of insurance reimbursement.
• Conduct discharge assessments and update patient records in real-time.
• Utilize age-appropriate teaching methods for patient and family education.
• Assume case management responsibilities, prioritizing care needs and revising care plans as necessary.
• Participate in interdisciplinary team meetings and communicate progress toward care goals.
• Ensure a safe environment for patients and coworkers, following infection control measures.
• Participate in monthly staff meetings and contribute to quality improvement programs.
• Provide orientation and mentorship to new employees, demonstrating a willingness to teach.
  
  

Required Experience / Certifications / Licensure

• Must have 2-3 years of experience as a Registered Nurse in a home hospice environment.
• Current New Jersey RN License.
• Valid New Jersey driver's license.
• BLS certification is required.
• Familiarity with EMR systems and strong critical thinking and communication skills.
