Job Description
The Registered Nurse (RN) will provide compassionate and competent care in a home hospice setting. Responsibilities include performing patient assessments, developing care plans, and ensuring the effective utilization of services while communicating with interdisciplinary teams.
Responsibilities
- Perform patient assessments and determine the appropriateness for hospice care.
- Develop and implement patient and family care plans.
- Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement.
- Complete discharge assessments and maintain thorough documentation.
- Utilize age-appropriate teaching methods and document patient education tools.
- Assume case management responsibilities and prioritize patient care needs.
- Ensure compliance with hospice regulations and demonstrate knowledge of pharmacology.
- Participate in performance improvement activities and assist in orienting new employees.
- Minimum of 2 years of experience as an RN in a home hospice environment.
- Current New Jersey RN License.
- Valid New Jersey driver’s license.
- BLS certification required.
- Experience using an EMR, with strong critical thinking and communication skills.
