Pay: $80,000.00 – $105,000.00 per year

Atlantic Physical Therapy Center is a Physical Therapist–owned and operated practice dedicated to delivering exceptional care in both clinic and community-based settings. We are expanding our Home Health Part B services and are seeking a motivated, compassionate Home Health Physical Therapist to join our growing team.

This role is ideal for a PT who enjoys autonomy, one-on-one care, and forming meaningful patient relationships in the home environment—while still being connected to a supportive, therapist-centered organization.

What You’ll Do

• Provide high-quality Medicare Part B physical therapy services in patients’ homes
• Conduct thorough evaluations and develop customized treatment plans
• Deliver evidence-based interventions to improve mobility, strength, balance, and overall functional independence
• Communicate and collaborate with referring providers, caregivers, and the clinical team
• Maintain appropriate documentation consistent with home health Part B requirements

What We’re Looking For

• Doctor of Physical Therapy (DPT), Master’s or Bachelor’s in Physical Therapy
• Active New Jersey PT license (or eligibility)
• Prior home health or outpatient experience preferred (Part B experience a plus)
• Strong clinical reasoning and patient-centered approach
• Excellent communication and time-management skills
• Ability to work independently in the home environment
