Job Description
The Community Health Registered Nurse (PD RN) will implement the Community Health department’s clinical and outreach initiatives, connecting and engaging with patients and community members through health and wellness events, screenings, flu shot clinics, and education programs. This role provides direct clinical support for community-based health services, assists patients in navigating care and resources.
Provide clinical oversight at community-based health events, including blood pressure, glucose, cholesterol, and other health screenings and flu shot clinics; ensure safe practices and adherence to AHS protocols.
Educate community members on preventive health, chronic disease management, and healthy lifestyle behaviors using evidence-based practices and culturally appropriate communication.
Support care coordination by identifying abnormal results, providing health counseling, and facilitating referrals or follow-up care through collaboration with AHS clinical departments and community agencies.
Serve as a clinical resource to Community Health team members and volunteers by reviewing protocols, offering guidance, and ensuring appropriate documentation and follow-up.
Ensure accurate documentation of flu vaccinations in Epic and maintain compliance with all data privacy and reporting standards.
Assist in the development and review of clinical policies, procedures, and educational materials to ensure quality, safety, and alignment with evidence- based guidelines. Maintain professional licensure, certifications, and competency through ongoing education and participation in department and system training initiatives.

Experience with electronic health records (Epic preferred). Excellent clinical assessment, communication, and interpersonal skills.

Strong organizational skills and ability to multitask effectively. Proficient in Microsoft Office applications
Qualifications
Graduate of an accredited School of Nursing; BSN preferred.
3-5 years of nursing experience required; experience in community health, public health, or ambulatory care preferred.
CPR certification required.
Valid NJ Registered Nurse License. NJ Driver’s License required.
Bilingual preferred.
