Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a full-time hospice Registered Nurse to work 8AM- 4PM in the Sussex County area, including weekends and on-call as required by department. In this position you will work with adult and geriatric patients of all backgrounds who require care in their homes. You are compassionate, thoughtful, and comfortable working with elderly and terminally ill patients. You will be responsible for managing a heavy caseload (up to 4 patients a day); time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs.

Entry level offers start at $43.00 / hour and increase based upon experience. Mileage reimbursement ($.70) included.

Internal referral bonus: $7,5000

Reports to: Assistant Manager

Required

  • Current NJ RN License
  • Valid and unrestricted driver’s license with a clean driving record
  • Associates Nursing Degree
  • BLS


Preferred

  • Bachelor’s degree in nursing.
  • All ASN must complete their BSN studies within five years of their start date.
  • Master’s degree in nursing or related field.
  • 1 year of experience in Nursing or relevant area.
  • ACLS certification.


Duties & Responsibilities

Performs Patient assessments.

  • Accurately assesses patient’s appropriateness for hospice care, learning needs and progress toward goals
  • Develops the plan of care for the patient and family


Ensures appropriate utilization of services

  • Maintains visit productivity standards
  • Demonstrates knowledge of insurance reimbursement


Completes discharge assessments

  • Performs laptop transfer of data twice daily
  • Completes admission assessment and transfers within 24 hours
  • Completes re-visit documentation and transfers


Uses age appropriate teaching methods for patients and families

  • Evaluates teaching plan and revises as necessary
  • Documents patient education tools used


Assumes responsibility for Case Management.

  • Develops/implements/evaluates plan of care based on evidence-based knowledge
  • Revises care plan as indicated
  • Prioritizes care needs
  • Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines
  • Shows evidence of interdisciplinary communication
  • Performs HHA supervisions per standards
  • Ensures appropriate utilization of services
  • Follows scheduling process
  • Communicates progress toward goals with IDT
  • Attends and participates in IDT meetings every 2 weeks
  • Evaluates the plan of care and revises as needed


Maintains safe environment for patients and co-workers

  • Follows infection control measures and reports infections
  • Demonstrates knowledge of pharmacology and safe medication management
  • Routinely reconciles medications
  • Maintains current medication orders


Attends monthly staff meeting

  • Participates in PI meetings and activities/committees as requested.
  • Accepts other assignments as requested


Assumes responsibility for case management.

  • Prioritizes schedule to accommodate patient's needs
  • Documents case management activities.
  • Demonstrates knowledge of pharmacology and safe medication management
  • Routinely reconciles medications. Maintain current medication orders.


Participates in Performance Improvement Program

  • Reviews health records and gathers data to support PI activities
  • Attends UR meetings as requested
  • Provides orientation/ mentorship to new employees
  • Cross trains to learn new skills and demonstrates a willingness to teach other


Assists with growing agency business

  • Provides orientation to new employees
  • Accepts other assignments as requested
  • Cross trains to learn new skills

We’re accredited by the Joint Commission and licensed by the New Jersey Department of Health. Atlantic Visiting Nurse - Home Health services are available to residents of Morris, Union, Essex, Sussex, Somerset and Warren counties in New Jersey.

Hospice

At Atlantic Visiting Nurse, we believe that those living with advanced illnesses should spend their final days in comfort and peace. Our compassionate, specialized care is designed to minimize patient suffering and to support families and loved ones in their roles as caregivers. As one of the first hospices in New Jersey and the United States, we have more than 40 years’ experience serving private homes, nursing facilities and assisted living facilities in Essex, Morris, Somerset, and Union counties. When patients and their primary care physicians agree that curative treatment is no longer appropriate or desired, a referral to Atlantic Hospice may be appropriate. Our hospice team works with referring doctors to develop individualized care plans that manage pain, control symptoms, and ensure dignity and respect in the final days of life. We also have nurse liaisons at Morristown Medical Center, Overlook Medical Center, Newton Medical Center, Chilton Medical Center, and Hackettstown Medical Center, as well as other health care facilities in the area, to answer questions and ease the transition to end-of-life care.
