Job Title: Forensic Transition Manager- Atlanta

Pay Grade: L

Workplace: Hybrid- 3 days per week in office

Travel: 25%

Job Summary:

The Forensic Transition Manager will serve to provide transitional support to the Forensic Diversion Manager. In addition, he/she will manage a reduced caseload of special population cases.

 

Job Duties include, but not limited to:

 

·       Facilitate movement of incarcerated/hospitalized/community individuals to the appropriate level of mental health care

·       Provide orientation and ongoing consultation, when needed, on procedural aspects of Forensic Transition Coordinators (FTC) responsibilities to FTCs and DBHDD staff.

·       Provide clinical oversight and serve as supervisor to FTCs

·       Audit and provide feedback, and consultation with Forensic Community Coordinators on timeliness and quality of documentation of community monitoring/competency restoration/remediation services provided by FCCs

·       Feedback to Community Forensic Directors and State Office regarding status of FTC/FCC services and training needs

·       Manage statewide database to track community-based competency remediation in Juvenile Court and competency restoration in State and Superior Courts to ensure that services are initiated and re-evaluations are completed in a timely manner.

·       Manage database of forensic individuals in Community Integration Homes and Forensic Apartments and collaborate with the Community Clinical Coordinator to facilitate timely transfer to new levels of care when appropriate.

·       Training of new and existing FTCs/FCCs in the identification of and coordination of community-based services.

·       Identifying viable community behavioral health treatment options for State and Superior Courts seeking treatment as alternatives to incarceration.

·       Develop and maintain a working relationship with other agencies in the community to stay current about available resources.

·       Develop and maintain working relationships with Forensic State Office Team (CIH Clinical Coordinator, CIH Behavioral Health Counselors, and Forensic Housing Administrator).

·       Serve as liaison for community programs to facilitate service acquisition for individuals

·       Providing case management and educational sessions focused on competency remediation for individuals ordered by the State, Superior or Juvenile Court to receive these services in the community.

·       Provide educational training to individuals mandated to outpatient competency remediation services.

·       Alert forensic psychologist assigned to the case of competency status.

·       Provide documentation of sessions and other information to evaluating forensic psychologist.

·       Facilitate referrals to other behavioral health treatment programs if needed.

·       Providing case management and oversight of individuals who have been adjudicated Not Guilty by Reason of Insanity (NGRI) and conditionally released or have been adjudicated Not Competent to Stand Trial and Not Restorable (IST-NR) and released on civil outpatient commitment/ bond with mental health monitoring.

·       Provide reports to the respective Hospital/ Community Forensic Director(s) regarding timely movement of individuals to the next level of care.

·       Track and schedule dates for Court updates. Coordinate with forensic psychologist to complete status update reports for the Court.

·       Attend court hearings, as needed.

·       Participate at hospital recovery team meetings as required.

·       Participate in community transition meetings as needed.

·       Other duties as assigned.

 

Supervises, guides, and/or instructs the work assignments of subordinate staff.

Supervises and plans work of assigned staff. Assesses needs of target population.

Compiles reports of findings and submits in appropriate format.

Coordinates program with other health care providers/organizations to promote optimal implementation of program services.

Develops key performance indicators for staff and monitors achievement.

Provides input regarding policies, procedures, protocols, and guidelines for the program.

Provides technical assistance and advice to develop, implement, and improve programs related to specialty.

Research and recommend best practices. 

Reviews and interprets state and federal regulations related to the program.

Other duties as assigned.

 

Preferred Qualifications:

·       Master's degree in Psychology, Counseling, Social Work, or related field.

·       Licensure in Social Work or Professional Counseling.

·       Four years of experience working with individuals with mental illness or developmental disabilities.

·       Knowledgeable about the criminal justice system, court proceedings, and community mental health treatment providers.

 

Preferred Qualifications:

Knowledgeable about forensic statutes, risk management for public safety, and state hospital procedures.

FOR THIS TYPE OF EMPLOYMENT, STATE LAW REQUIRES A NATIONAL AND STATE BACKGROUND CHECK AS A CONDITION OF EMPLOYMENT.  

Bachelor's degree in a related field from an accredited college or university and five (5) years of experience performing work related to the area of assignment, which includes two (2) years in a managerial or supervisory role or one (1) year of experience at the lower level Healthcare Prgm Consultant 3 (HCP092) or position equivalent.
