POSITION: Spiritual & Bereavement Care Coordinator REPORTS TO: Administrator
STATUS: Exempt Non-Exempt
POSITION SUMMARY: The Spiritual & Bereavement Care Coordinator, is responsible for developing a comprehensive bereavement program ensuring that patients and families receive necessary bereavement support. Additionally, the
Spiritual & Bereavement Care Coordinator provides Spiritual Assessments, Spiritual Care, and Clergy Services, for patients and families. The Spiritual & Bereavement Care Coordinator assists company staff in managing their grief.
The responsibilities of a Spiritual & Bereavement Care Coordinator revolve around caring for the spiritual needs of each hospice patient and their families. Your duties may include arranging counseling for patients and grieving families, offering spiritual support to caregivers, and helping with funeral planning.
PRINCIPAL ACCOUNTABILITIES:
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Spiritual Care
1. Provide direct spiritual support and/or counsel to patients/families in keeping with patients’/families beliefs;
2. Provide short-term pastoral counseling to patients, families and staff, using age appropriate communication;
3. Serve as a member of the Interdisciplinary Team involved in client care planning, with particular focus on spiritual components of care;
4. Facilitate clients’ and families’ access to spiritual care as requested and desired; Inform clients and their families about available community clergy;
5. Demonstrate competency in the cultural and age specific needs of the clients;
6. Provide or facilitate rituals at times of transition, including death.
Bereavement
1. Ensures the company Bereavement Policy and Procedure is followed and company is complaint with state and federal regulations associated with Bereavement services.
2. Upon admission, identifies bereavement risk, goals and interventions, performing an initial assessment of patient and the patient’s family and/or caregivers.
3. Provides patient and family and/or caregiver with information about company services and support including the availability of clergy.
4. Upon patient’s death, for the family member(s) and/or caregivers(s), performs a post death bereavement assessment and develops a bereavement Plan of Care based on identified needs.
5. Ensures that the bereavement Plan of Care is followed and provides ongoing bereavement assessment.
6. Assumes and maintains accountability for bereavement program compliance with federal and state regulations.
7. Provides bereavement training for volunteers.
8. Facilitates or participates in grief support groups.
9. Conduct memorial services/liturgical assignments for clients who have died providing continuity of spiritual support when requested.
10. Conduct or make arrangements for funeral or memorial service when indicated. Plans and conducts memorial service for bereaved families as requested and at least annually.
11. Has a duty to report to the assigned RN, any changes in patient condition and anytime the patient is in pain.
12. Has a duty to report any potential misconduct or violation of Hospice Standards of Conduct or federal or state laws and regulations.
Administration
1. Develop and supervise the provision of spiritual care to clients and families and assist in design, implementation, and evaluation of the spiritual care program, ensuring proper documentation.
2. Develop and maintain a resource group of clergies to whom specific aspects of spiritual care may be delegated.
3. Process spiritual assessments and referrals for assignment to professional, staff, volunteers and spiritual care volunteers as requested according to need, level of expertise required, and staff available.
4. Arrange for on-call availability of Spiritual Care services.
5. Facilitate and advocate for the cultural needs of patients, families and staff.
Education
1. Work with staff, volunteers, clergy, lay ministers and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss.
2. Provide education, support and consultation to clergy in the community regarding end of life issues;
3. Participate and/or lead discussions on ethics related to end of life care;
4. Integrate spiritual care competencies into policies for staff and volunteers in coordination with the Volunteer
Coordinator.
General Responsibilities
1. May be required to perform the responsibilities of the Bereavement Coordinator. See the job description for the Bereavement Coordinator.
2. In coordination with the Director of Volunteer Services recruit train and supervise spiritual care and bereavement volunteers.
3. Conduct memorial services/liturgical assignments for clients who have died providing continuity of spiritual support when requested and when schedule permits.
4. Conduct or make arrangements for funeral or memorial service when indicated;
5. Establish a working relationship and support all volunteer Spiritual Care providers;
6. Other duties as assigned.
7. Assesses the need for and provides spiritual care services for patients and their families.
8. As a member of the IDG/IDT, assists in the development and implementation of the Plan of Care.
9. Documents all visits with patients and families according to Hospice policy, procedures and guidelines.
10. Interacts with the patient’s own clergy as needed.
11. Interacts with religious community advising of hospice program and benefits.
12. Is available to attend to families when needed at time of death which may occur, from time to time, after hours.
13. Develops and maintains a list of clergy from various denominations who are available to work with hospice patients and families.
14. Performs on-call duties as required.
15. Adheres to Hospice Standards of Conduct.
16. Has a duty to report any potential misconduct or violation of federal or state laws and regulations.
17. Has a responsibility to participate in staff training and education programs.
18. Performs other duties as assigned.
MINIMUM QUALIFICATIONS:
1. One year of experience dealing with patients undergoing significant stress, the dying process, or grief preferred. Previous hospice experience preferred.
2. Graduation from an accredited seminary or other training program, ecclesiastical endorsement, and/or ordination.
3. Bachelor’s degree in theology or related field preferred.
4. One Clinical Pastoral Education certification .
5. Spiritual Care Coordinators must have documentation of ordination or commission by a recognized faith group and completed at least one unit of Clinical Pastoral, Education (CPE) from a nationally recognized provider.
6. Good understanding of dying, death and the grief process.
7. Ability to provide nondenominational spiritual care to patients/families from varied religious backgrounds.
8. Excellent communication and customer service skills.
9. Reliable transportation, current driver’s license and current auto insurance.
10. Must pass the following including but not limited to a background check, OIG exclusions check, license verification, reference check and review of social media.

(1) Bereavement counseling. The hospice must:
I. Have an organized program for the provision of bereavement services furnished under the supervision of a qualified professional with experience or education in grief or loss counseling.
II. Make bereavement services available to the family and other individuals in the bereavement plan of care up to 1 year following the death of the patient. Bereavement counseling also extends to residents of a
SNF/NF or ICF/IID when appropriate and identified in the bereavement plan of care.
III. Ensure that bereavement services reflect the needs of the bereaved.
IV. Develop a bereavement plan of care that notes the kind of bereavement services to be offered and the frequency of service delivery. A special coverage provision for bereavement counseling is specified in §
418.204(c).
(2) Spiritual counseling. The hospice must:
I. Provide an assessment of the patient's and family's spiritual needs.
II. Provide spiritual counseling to meet these needs in accordance with the patient's and family's acceptance of this service, and in a manner consistent with patient and family beliefs and desires.
III. Make all reasonable efforts to facilitate visits by local clergy, pastoral counselors, or other individuals who can support the patient's spiritual needs to the best of its ability.
IV. Advise the patient and family of this service.
