Key Responsibilities
- Complete comprehensive wound assessments and develop individualized care plans
- Prescribe medications and treatments as clinically appropriate
- Provide in-person and telehealth visits (as permitted by state regulations)
- Document patient care thoroughly and accurately
- Deliver care in patient homes, with occasional visits in SNFs or rehab facilities
Qualifications
- Active Nurse Practitioner (FNP, APRN) or Physician Assistant license in Texas
- Master’s degree from an accredited NP/PA program
- Preferred: 1 year experience in primary care, internal medicine, urgent care, or wound care
- Preferred: ER or ICU experience
- Strong clinical judgment, communication, and organizational skills
- Compassionate, self-motivated, and patient-focused
Schedule
- Monday–Friday, daytime hours only
- No evenings, weekends, or on-call shifts
