As a vital member of our Athena Home Health & Hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.

As a vital member of our Athena Home Health & Hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.
Responsibilities:
- Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing process
- Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations
- Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions
- Counsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriate
- Act as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate patient care for assigned caseload
- Graduate of an accredited school of nursing
- One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care
- Active and unencumbered RN license in Connecticut
- CPR certification
- Bachelor's degree, with one (1) year of hospice care experience preferred
