Athena Home Health & Hospice is looking for a Clinical Liaison to join our field team!
We service hospice patients throughout Connecticut. This is an on-the-road position that requires the candidate to have an active driver's license and insured and reliable vehicle. Salaried role with weekly pay.
You are responsible for driving census growth in the market through relationships and clinical expertise. You are the bridge between our referral partners and the hospice team.

You are responsible for driving census growth in the market through relationships and clinical expertise. You are the bridge between our referral partners and the hospice team.
Requirements:
- Must possess an active and unencumbered clinical license in Connecticut (RN, LPN, or Social Worker preferred).
- A minimum of two (2) to three (3) years in healthcare experience required (hospice, home health, or related field).
- Experience in sales, marketing, and/or business development preferred.
- Strong knowledge of hospice eligibility and disease progression.
- Must possess an active driver’s license and have reliable and insured transportation.
- Must be able to read, write, speak, and understand the English language.
Duties:
- Act as a liaison between referral sources and the hospice team to identify and develop new referral opportunities.
- Develop and maintain daily and monthly marketing and account management plans.
- Drive census growth through effective sales calls, referral management, and relationship building.
- Perform clinical evaluations (“soft screens”) to determine hospice eligibility.
- Ensure timely referral-to-admission conversion in collaboration with intake and clinical teams.
- Plan and deliver in-services and educational programs (Hospice 101, disease progression, symptom management).
- Customize education based on clients’ needs and knowledge gaps.
- Participate in care conferences and case discussions to reinforce clinical partnership.
- Maintain strong relationships with key decision-makers (Administrators, Social Workers, etc.).
- Track and report referral activity, admissions, and educational sessions.
- Ensure compliance with Medicare CoPs and regulatory standards.
- Participate in orientation and ongoing training programs.
- Maintain knowledge of competitors and market dynamics.
