GENERAL PURPOSE: 

The Clinical Care Coordinator performs high-level administrative and operational coordination functions essential to the management of agency operations. The primary duty includes exercising independent judgment and discretion in planning, coordinating, and managing scheduling activities, workflow processes, and communication systems that directly impact patient care delivery and organizational effectiveness. This role is responsible for analyzing scheduling needs, resolving operational challenges, prioritizing competing demands, and implementing solutions that support business operations. The position operates with minimal supervision and requires decision-making authority on matters of significance.

QUALIFICATIONS:

  • High school diploma required.
  • Must have a valid driver’s license.
  • 3 years of general office experience; knowledge of medical terminology preferred.
  • Excellent computer skills required, excellent interpersonal and organizational skills.
  • Excellent phone skills and interpersonal/ communication skills needed. 
  • Bilingual preferred, but not necessary.
  • Reliable transportation. Valid and current auto liability insurance.

ESSENTIAL JOB FUNCTIONS:   

 

  • Independently plans, coordinates, and oversee daily scheduling operations to ensure efficient patient service delivery and optimal staff utilization.
  • Evaluates staffing patterns, patient needs, and operational priorities to make real-time scheduling decisions and adjustments.
  • Resolves complex scheduling conflicts using judgment and knowledge of agency operations, patient requirements, and staff capabilities. 
  • Determines prioritization of work assignments based on organizational goals and service demands.
  • Monitors and analyzes scheduling data and operational metrics to identify trends, inefficiencies, and improvement opportunities. 
  • Exercises discretion in handling sensitive and confidential information, including patient, employee, and operational data.
  • Make independent decisions regarding schedule modifications, staff coordination, and operational problem resolution.
  • Interprets policies and apply them to varied operational situations without immediate supervision.
  • Serves as a key resource for staff regarding scheduling protocols, operational procedures, and system usage.
  • Acts as a central liaison between patients, clinical staff, referral sources, and leadership.
  • Facilitate effective communication across departments to ensure continuity of care and operational consistency.
  • Addresses and resolves service-related concerns by evaluating circumstances and determining appropriate responses.
  • Recommends and implements process improvements that impact service quality and organizational performance.
  • Facilitate the communications system which may include, mail distribution, pagers, phone systems, memos, answers incoming calls, documents and disseminates appropriate messages.
  • Provides clerical support which may include preparation of admission, orientation, and new hire packets, typing, special projects, filing and copying. 
  • Provides effective communication to patients/clients, staff members, other health care professionals and referral sources.
  • Provides computer support and data entry, as needed.  
  • Competently handle multi-line phone system.  Demonstrates understanding of system features.  
  • Demonstrates commitment, professional growth, and competency. 
  • Promotes the Agency philosophy and administrative polices to ensure quality of care. 
  • Files and keeps necessary data. 
  • Meets attendance and punctuality expectations. 
  • Consistently adheres to agency dress code. 
  • Other duties as assigned.

Physical and Sensory Requirements (With or Without the Aid of Mechanical Devices):   

   

Walking, reaching, climbing, bending, lifting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smell and temperatures, ability to read and write, ability to communicate with personnel, patients, and outside agencies. Ability to present to groups and the ability to remain calm under stress.
