Community Health Worker – Remote with Local Travel
Join our team as a Community Health Worker focused on the Gate to Success program. In this role, you will act as a bridge between healthcare services and the community, helping members overcome barriers to care, education, and employment. You will work mostly from home while traveling locally to meet members and attend community events.

Key Responsibilities
- Perform outreach to members via phone and in person to address health gaps
- Conduct social health assessments and identify barriers like transportation or childcare
- Coordinate with providers and community agencies to secure member services
- Empower members to take an active role in their health and career paths
- Represent the organization at local community events
Requirements
- High School Diploma or GED 1 year of experience in community health or a related field
- Lived experience in the community served
- Must reside in one of the following PA counties: Allegheny, Armstrong, Beaver, Bedford, Blair, Butler, Cambria, Fayette, Greene, Indiana, Lawrence, Somerset, Washington, or Westmoreland
- Ability to obtain PA CCHW Certification within 12 months of hire
- Must be a U.S. citizen
Schedule
- Standard business hours Monday through Friday
- Occasional evening or weekend availability for community events
