Job Overview

• Title: Licensed Social Worker (MSW) – Adult Day Care (Temp-to-Perm)

• Department: Adult Day Care – Social Work (Everett)

• Schedule: Days, 3x8 (08:00–17:00); Mon–Fri; no weekends; no on-call; paid inter-site travel/mileage during workday (commute not paid)

• Start Date: 10/06/2025

Job Description

• Summary:

  • Provide social work services for geriatric participants in an Adult Day Care setting; primarily center-based with moderate/light home assessments; complete comprehensive psychosocial evaluations; contribute to integrated, person-centered care plans; coordinate community/financial resources; ensure continuity across settings; collaborate closely with the interdisciplinary team.

• Responsibilities:

  • Perform initial/ongoing psychosocial assessments and reassessments

  • Provide one-on-one and group counseling; facilitate support/resources

  • Develop and document individualized, integrated plans of care with IDT

  • Coordinate community and financial resources (Medicare/Medicaid/State)

  • Conduct select home visits within Snohomish County; self-schedule within 8–5

  • Maintain timely, accurate EMR documentation (Epic preferred)

  • Uphold Health System mission, policies, and applicable standards

• Key Requirements:

  • 2 years’ post-graduate social work experience with geriatric population

  • Adult Day Care/LTC/community health experience strongly preferred

  • EMR proficiency

  • Epic preferred

  • Valid driver’s license, proof of insurance, and reliable transportation

  • First-time travelers accepted; locals welcome

  • RTO must be disclosed at submission; subject to manager approval

Qualifications

• Education:

  • Master of Social Work (MSW) required

• Experience:

  • Minimum 2 years with older adults (Adult Day Care, LTC/SNF, community health, hospital case management, or similar)

• License:

  • One active Washington social work license required: LICSW, LSWAIC, or LASW

  • BLS required

• Skills:

  • Psychosocial assessment; interdisciplinary care planning; community/benefits navigation

  • Counseling, crisis intervention, motivational interviewing (preferred)

  • EMR/Epic documentation; strong communication and cultural competence

  • Time management; autonomous scheduling; safe driving for inter-site travel
