Overview
We are seeking a skilled and dynamic Wellness Nurse to provide periodic wellness clinics at our independent living location and to provide home care staff education and client assessments in the community, including care plan development.
In this impactful role, you will deliver comprehensive nursing care, support health education initiatives, and collaborate to enhance the quality of life for individuals across various care settings of our continuum.
This is a flexible PRN/per diem position with fluctuating hours based upon caseload.
Responsibilities
- Conduct thorough client assessments and plans of care development documenting using electronic health record (EHR) systems.
- Administer medications and injections as may be prescribed ensuring adherence to HIPAA guidelines.
- Educate clients and their families on health coaching topics including medication administration, nutrition, disease management strategies and home safety.
- Conduct Wellness Clinics for clients periodically including blood pressure, pulse oximeter and general education on topics related to aging.
- Provide staff education on policies and procedures, home care certifications, and attend outside wellness fairs.
Qualifications
- Current MA nursing license; experience in home care settings preferred.
- Demonstrated proficiency with electronic medical records documentation.
- Extensive background working with older adult populations helpful.
- Experience with teaching preferred including CPR, home care ADL tasks, and first aid.
- Strong understanding of home care policies and procedures, and client assessment skills.

Pay: $30.00 - $40.00 per hour
