Home Care Nurse- Training School and Field Assessments

At Home Solutions is looking for a Home Care RN in the Buffalo area to provide high-quality patient care in the home as well as caregiver training and education. The RN must be able to travel to patients' homes to complete required assessments and administer healthcare services.

Duties and Responsibilities:

• Conduct ongoing patient care and assessments.
• Initiates Plan-of-Care and makes necessary changes as necessary.
• Prepares written documentation and summaries of the patient’s status and progress as needed to continue home health services.
• Administer PCA classes and Inservices as needed
• Maintain close communication with the Director of Patient Services and the interdisciplinary team.
• Responds to phone calls and emails in a timely manner.
• Reporting all issues and concerns to the Director of Patient Services immediately.
• Completes documentation accurately and submits timely to the agency
• Provides the highest quality care in accordance with DOH regulations.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Administer Physician-prescribed medication.
• Dress or redress wounds and assess the healing progress.
• Provide education to patients and families on proper home health care procedures and strategies.

Qualifications

• Graduate of an approved school of nursing
• Current NYS RN license
• Liability insurance is required
• Excellent communication skills, detail-oriented, patient care, and assessment skills
• Able to function independently with minimum supervision
• Demonstrates strong critical thinking skills
• Ability to travel to patients’ homes
• Long-term care 1 year preferred
• Nursing home care 1 year preferred
• Rehabilitation center 1 year preferred

Schedule:

• 9am to 5pm Monday - Friday

Job Type: Full-time

Pay: $45.00 - $50.00 per hour
