Registered Nurse or LPN

We are seeking dependable and compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our team on a per diem basis. Candidates should have experience and knowledge of home care services and be comfortable providing high-quality care to patients in their homes.

Responsibilities include:

• Providing safe, compassionate, and professional nursing care in the home setting
• Performing nursing assessments and monitoring patients’ conditions
• Administering medications and treatments as ordered
• Following individualized care plans and physician orders
• Communicating effectively with patients, families, physicians, and the care team
• Completing all required clinical documentation accurately
• Recognizing changes in patients’ conditions and reporting concerns appropriately
• Maintaining patient confidentiality and complying with all applicable policies and regulations
• Providing patient and family education as appropriate
• Maintaining professional and respectful relationships with patients and their families

Qualifications include:

• A current, active RN or LPN license in the applicable state
• Previous home care or home health experience strongly preferred
• Knowledge of home care practices, documentation, and patient safety
• Strong clinical, communication, and organizational skills
• The ability to work independently in a home care environment
• Reliability, dependability, and compassion
• Reliable transportation and the ability to commute to patients’ homes
• The ability to travel within the assigned service area as needed
• Current CPR certification preferred
• Successful completion of required background and employment checks

Pay: $35.00 - $45.00 per hour
